Quick Consult consultation for the NHS

The Health & Social Care Act requires all general practices to become members of a Clinical Commissioning Group (CCG). CCGs will manage the commissioning of NHS healthcare from April 2013, when Primary Care Trusts and Strategic Health Authorities become defunct. The Department of Health has published a model constitution and recommends that CCGs incorporate its guidelines into their aligned practices.

As a part of this incorporation, NHS Swindon Shadow Clinical Commissioning Group used Quick Consult to run a one-off survey. The consultation was aimed at getting feedback and insights from practitioners and stakeholders about the proposed structure and constitution of the Shadow CCG.

The survey ran from 21 May to 10 June 2012, and had 50 responses. The survey was created by the Delib team in collaboration with the Chair of the Shadow CCG, Dr Peter Crouch. The questions included:

  • How happy are you with our proposals for the new Clinical Commissioning Group?
  • How well do you think the proposals reflect the views of local people?
  • Do you think this plan is enough of a shift towards GP-led commissioning?
  • How happy are you with the balance of different clinicians on the Board?
  • Do you think the proposed CCG is sufficiently transparent?
  • Are you happy with the arrangements for handling conflicts of interest?
  • Do you have any suggestions for improving our plans?


The engagement exercise revealed that approximately 80% of respondents were either ‘very happy’ or ‘happy’ with the proposed changes. Around 78% of people felt the proposal reflected the views of local people. The full report is available here. The results will help Swindon CCG move forward with confidence in their plans.

Thoughts on consultation practices in the NHS

Here at Delib we are intensely interested in the way others are consulting their stakeholders, nothing makes us happier then those practising digital democracy well.

I have recently been working with the NHS to assist with the grand ideal which is Digital Democracy, and came across various methods which are used. I thought this may interest others involved in social media and online consultations so thought I would share my findings with you all.

Many of the health organisations I have spoken to are using methods of engagement which take place in the community; Forum Groups are a good example of this. These are a wonderful ideas but in practice this does alienate those who are unable to physically attend.

I have also seen online membership bodies used, which allow a more relaxed approach to patient participation. Once again there is a problem where potential users are forced to sign up and apply to join. In practice this enables demographics of the population to be monitored by the organisers, but can be intimidating for those of us who wish to be active in having their say without attention being drawn to them.

Doctors surgeries have recently ‘upped the ante’ with Patient Participation Groups trying to engage with their patients through new methods. Many are done through emails, however coffee mornings are rather popular too. Rotherham NHS have been letting the CCG’s in their areas use their consultation hub. They found that this solved the problem of patients not being able to attend meetings and widening the scope of participation by allowing them to register their opinions remotely.

These are just a few of the ways we’ve seen health organisations engaging their residents and the problems they have faced along the way. We constantly take these learnings on board trying to ensure all our apps help to remedy as many of these problems as possible by allowing a wider pool of stakeholders to be engaged.

Future of Stakeholder Engagement in the NHS

Here at Delib we like change, and we like democracy. With the NHS arguably experiencing it’s greatest overhaul since it’s conception in 1948, I thought I would share some ideas for engaging patients and other stakeholders in 2012.

The NHS Operating Framework puts patients at the centre of decision making with their experience of health and supporting care services central to the drive for further improvements. Patient engagement will be a key part of the new look NHS; add this to the emphasised ideals and healthcare professionals face rapidly approaching hurdles on the horizon.

New digital strategies can engage and involve patients and other stakeholders, proving a cost effective addition to traditional methods, with the ability to reach some groups that wouldn’t be reached by traditional means. Lambeth Health and Wellbeing Board has recently been delving into this with gusto, as covered here by Guardian Healthcare.

The Department of Health has also been using digital engagement to reach stakeholders across the country, and the current Trusts and PCT’s are linked to this consultation hub (provided by us using Citizen Space – hassle-free software for managing and running all consultation and engagement activity across your organisation).

Meanwhile, although long-established (1978), the National Association for Patient Participation (NAPP) has great case studies and practical best practice advice for GPs, particularly relevant since a General Medical Services contract change in April 2011 requires GP practices to involve patients and undertake local surveys.

Five useful links for the new look NHS

I speak to a lot of health care professionals about stakeholder engagement. The overwhelming problem faced by many in the NHS is the new engagement strategies needed by the GP Consortiums. I’ve gathered some helpful links to assist those re-thinking their Public and Patient Engagement strategies.

With all the uncertainty in the current state of health reforms, it is good to know that the NHS alliance is championing the cause of the GP Consortium.

A very real concern of many health care professionals is how to engage all stakeholders, something Journalista has blogged about very well.

The Guardian have published a rather interesting article about the NHS use of Twitter and Facebook, which is well worth a read.

The guys at RCGP Centre for Commissioning have developed a ‘Rough Guide to Engagement and Experience for GP Consortia’ on the emerging national framework governing patient interaction. As a starters guide it’s invaluable.

NHS Social Media – my favourite find of last week, this is a straightforward twitter discussion for anyone involved in using social media in the NHS. The discussion takes place every Wednesday evening and is easy to join or watch. No webinar software to worry about, no forums to register for, just useful, practical discussion via Twitter.

Couple of interesting pieces I’ve run across
Interesting postGP practices ‘despondent’ as they struggle to meet terms of patient participation DES. I know there are NHS politics playing out in that post – but many practices *do* struggle to use the web effectively – for a host of reasons, including time & money, understanding what patients need from online resources, and understanding what is possible (disclaimer a number of my immediate family are GPs).

Stockport PCT choose Citizen Space as a key tool to help manage change within the NHS

With a major period of change in the NHS, many PCTs are having to increase consultation needs within a restricted budget. Having recently chosen to use Citizen Space, Stockport PCT have identified the system as a key tool to help them manage this change:

Prior to using Citizen Space , Stockport PCT found their teams were inadvertently asking local stakeholders similar questions, often risking ‘consultation fatigue’ amongst members of the public. The trust are now able to centralise consultation activity, providing visibility for their teams on all closed, current and forthcoming consultations, and helping avoid consultation fatigue.

Stockport PCT are currently updating their Consultation Policy, which outlines a clear process for undertaking a duty to consult, and Citizen Space will be ‘at the heart of this process’. Whenever changes to their services and policies are now proposed, staff can use Citizen Space to review what patients and the public have already told them.

Stockport PCT are also encouraging their GP practices, clinics and consortia to use the site to streamline their patient experience work.
